Entry of Appearance is required to be filed as a stand alone pleading called "Appearance" and all Answer fees shall be paid with the initial Appearance pleading.

Jury Demand is required to be filed as a stand alone pleading and the fee shall be paid at the time of filing said Jury Demand.
